New Patients

We are always welcoming new patients to the Practice. All you have to do is call into the surgery or complete our registration forms in our Registering Online section which you can bring to the surgery to make an appointment with our practice nurse for a new patient medical.

Also we ask if you could bring along some form of identification of your present address.

Online Registration

You can register with the practice online via Patient Access, by completing our online registration form linked below or by downloading, completing and returning the pdf forms linked below.

Please note once you have completed these forms you must call in or ring on 0191 417 3557 to make an appointment with our Practice nurse for a new patient medical; where possible could you also bring along a form of Identification with you to the appointment.

When you register you will also fill out a medical questionnaire, this is because it can take a considerable time for us to receive your medical records, when you come to the surgery you will be asked to sign this form to confirm that the details are correct.
