We are unable to issue covid vaccination passport certificates at the practice.

Please see www.gov.uk/guidance/demonstrating-your-covid-19-vaccination-status-when-travelling-abroad for further information.

Just like a normal passport, make sure to leave plenty of time before you set off on holiday to apply for your Vaccine Passport.

If you’re getting the #NHSApp for the first time, it can take up to (time) so that your identity can be verified.

If you’re not able to use the App, as long as you’ve had two doses of the vaccine, you can receive a paper copy by ringing 119 and one will be posted to you within five working days.

Proof of your vaccination status will be available on the NHSapp, the NHSapp can be downloaded for Iphone for android.

Alternatively, if you do not have access to a smartphone you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine, the letter may take up to 5 days to reach you.

More information regarding the NHSapp and our online services is available here.
